Coming in for the First Time?
Bring your photo ID, your insurance card if you have one, a list of anything you take regularly including supplements, and any recent lab work from another provider. If you do not have recent labs, that is fine. We run labs in house, so you are not sent across town and told to come back in two weeks.

Do You Take Insurance?
Some of what we do is covered by insurance and some of it is paid out of pocket. Hormone therapy, for example, is covered for a good number of our patients. Give us a call with your plan details and we will check it before you come in, so the cost is settled ahead of time instead of at checkout.
